chore(github): add a bug report issue form and chooser config ## What Adds the issue form DeckCraft needs before a prefilled "Report an Issue…" link can work, and a chooser config: - `.github/ISSUE_TEMPLATE/bug_report.yml`: bug form with the field ids `version`, `os`, `reproduction`, `expected`, `actual`, `document-info`, `system-info`, `diagnostics`. - `.github/ISSUE_TEMPLATE/config.yml`: blank issues stay enabled; adds the ArtCraft Discord as a contact link. The ids `version`, `os` and `system-info` are the ones PhotoCraft's open Report an Issue PR (storytold/photocraft#1312) prefills, and `document-info` is the one addition (file type only). Using
